Forms, other material furnished by commission and division
The division and commission make blank forms and other paperwork that are needed to run the workers' compensation system. These forms are free. They can also let employers and others print and hand out these forms.
287.630. Forms, other furnished by and . — The division and the commission shall prepare and furnish free of charge blank forms of all notices, , reports, proofs, and other blank forms and literature which they may deem proper and requisite to the efficient of this chapter. They may also authorize the and distribution of the blanks by employers and other persons.
