Delegation form, contents

Random law
In plain English

When a parent or legal guardian wants to give someone else authority to care for their child, they have to fill out a special form. That form must be signed in front of a notary public. It has to include the child's name and birthday, the name and contact info of the person receiving the authority, and the parent or guardian's name and signature. The form must state either that all parental powers are being handed over, or list only specific powers being given. No matter what, the form cannot give that other person the power to let the child get married, be adopted, have an abortion, or have parental rights taken away. The form also has to say how long the arrangement lasts and that the parent or guardian can cancel it at any time.
